MEDICATION FORM

Students who have prescribed or over the counter medication MUST have a separate medication form for each medication, signed by a doctor and submitted ASAP.

  • INFORMATION ABOUT SUBMITTING BOX TOPS
  •  * Box tops can also be collected for 6th Grade activities, please give them to your homeroom teacher.
    *Box Tops turned into the DC Club from 7th and 8th graders will only be used for the Washington DC Trip and will be sent to Worldstrides directly.
    * Box Tops must NOT be expiring (must have a date after November 1, 2018). Expired Box Tops will be removed before submission.
    *Box Tops MUST be taped to collection sheets (no glue or staples)
    * Be sure to include your name and grade on each sheet.
    * Box Tops submitted without a student's name attached to them or by someone not registered for the DC Trip will be
    applied to the DC Scholarship Account.

    CRITERIA FOR ATTENDING THE WASHINGTON DC TRIP

    1. Students must NOT be failing more than 2 classes for the year by the end of the 2nd Trimester.
    2. Students must maintain at least 130 total Viking Points by the end of the 2nd Trimester. Minimum of 65 points for Trimester 1 and Trimester 2 combined.
    3. Students must be in compliance with the district attendance policy. (No more than 10 unexcused absences for the year). Absences are ONLY excused if a written note is brought to the office within 10 days of the absence. Parent Calls alone DO NOT excuse an absence.
